1. PII Leakage 

PII leakage is the most immediate and severe AI risk in enterprise environments. It occurs when sensitive personal or customer data is exposed through prompts, outputs, logs, or third-party model processing. Because AI systems interact with unstructured inputs, leakage often happens unintentionally and at scale.

Mitigation starts with input validation, sanitizing user inputs to remove or neutralize injection attempts. Hardened system prompts reinforce boundaries that the model cannot override. 

Execution environments are sandboxed, limiting what agents can access or modify. Continuous behavior monitoring detects abnormal patterns that indicate attempted manipulation. 

Vendor Risk Management & AI Provider Due Diligence
